With a population of 395,000-400,000 residents, an estimated 650-800 people die from heart disease each year, equating to 54–67 deaths per month and two deaths every day
A large proportion of GP workload is from residents with heart diseases, creating huge demand and pressure on the NHS services
Residents living in deprived and underserved communities are disproportionately impacted, increasing health inequality

Clinical advice and health education
Free BP monitors for selected high-risk beneficiaries
Essential medications and signposting/referrals
